In laboratory and in vitro studies, dragon fruit pulp and especially peel extracts have been reported to show antioxidant activity, attributed to their betalains, phenolic compounds, and vitamin C. A 2021 review in Foods described the nutritional profile and bioactivities of red pitaya in detail, noting that the seeds and peel carry higher concentrations of polyphenols and betacyanins than the pulp and that these compounds are of interest as natural colorants and functional ingredients. A 2024 comparison in PeerJ analyzed nutrient composition and antioxidant activity across three cultivars, illustrating how pigment and antioxidant content vary considerably by variety. Human evidence is more limited and more mixed. A 2017 systematic review and meta-analysis in PLoS One examined dragon fruit and glycemic control, pooling a small number of randomized trials in people with prediabetes and type 2 diabetes. It reported a possible association with lower fasting glucose in the prediabetes group but no significant effect in the type 2 diabetes group, and the authors emphasized the small number of trials, modest sample sizes, and variability between studies.
